Enhancing Lives With Care

Residential Aged Care

Smiling GrandmaHunter Valley Care provides individualised care and supportive living environments for people who need assistance with daily activities and can no longer stay at home.

Even when faced with health challenges, people can still continue to live a quality life. With this in mind, we strive to enhance people’s lives by providing care for our residents in a way that celebrates their individuality and enriches their lifestyle.

We are passionate about empowering people in our care to thrive, and we understand that one way to help people maintain control of their life is by providing choices. Our residents have choices in all aspects of their lifestyle and living environment, from the type of accommodation and personal care services provided, to the health professionals and social opportunities that best suits their needs and preferences.

We encourage our residents to stay connected to their friends, family and community by providing opportunities that optimise lifestyle and social interaction. Stimulating activity programs are available to all residents of our Residential Aged Care facilities.

Our ageing in place philosophy ensures that our residents have the opportunity to receive a seamless transition of aged care services in line with their changing individual circumstances. Providing continuity of care within familiar surrounds – from low care through to high care and, when the time arrives, palliative care – gives our residents emotional security and peace of mind. They know that they won’t have to move just because their care needs increase.

Care types

Residential Aged Care provides assistance and supervision with personal care, activity programs, rehabilitation, hairdressing, beautician treatments, welfare and spiritual support.

Two levels of care are available in Residential Aged Care:

Low Level Care provides personal care services for people who need some assistance with daily living activities. Personal services such as meals, bathing, laundry, cleaning, administration of medications, and access to health care services are available, as well as a range of activities to enhance socialisation and promote health and wellbeing.

High Level Care provides personal care services for people who require 24-hour nursing assistance with their daily activities. Personal services may be the same as Low Care, but with more emphasis on nursing care for people with greater needs.

Extra Services

Extra Services are hotel-type services provided for an additional fee to enhance a person’s lifestyle. These services are available at all of our Residential Aged Care facilities and may include such benefits as a higher standard of accommodation, increased meal choices and additional activities. Our Admissions Officers will be able to give you details of the types of Extra Services available and the fees involved at each facility.

How to apply

The first step to moving into Residential Aged Care is to have a member of the Aged Care Assessment Team (ACAT) complete an assessment of the person seeking care. The ACAT member will meet at the candidate’s home or hospital and ask a range of questions to determine what level of care will best meet their needs.  The ACAT member will advise if a person is eligible to move into Residential Aged Care and whether low or high level care is required.

An ACAT assessment can be arranged through your doctor or local hospital. For details of your local ACAT team, call My Aged Care on 1800 200 422 or go to their website www.myagedcare.gov.au.

For more information on applying for Residential Aged Care and ACAT, please refer to the Department of Health’s How To Access Services Guide. This guide may also be provided by your doctor or local hospital.
